Patient-Centered Medical Home model

Examples of Acute Care Needs

Acute Bleeding

Injuries that will not stop bleeding require immediate medical attention to address the issue before the patients goes into shock. If you visit our clinic to address acute bleeding, we will first assess your injury to determine the appropriate treatment. Patients who require acute care to treat deep wounds may require stitches to close the wound and stop the bleeding.

Acute Injuries

Strains, burns, sprains and fractures can cause short and long-term issues if they are not treated. Treatment options may include bandages placed over wounds or burns to reduce your risk of infection and speed up the healing process or wraps, splints, or casts to immobilize limbs that have been injured and prevent the injury from worsening. Treatment may also involve prescribed medication to help relax your muscles and/or manage pain.

Digestive Symptoms

Patients who have persistent diarrhea and/or vomiting run the risk of developing complications, including dehydration. If you have had diarrhea or been vomiting for more than one or two days, our providers can identify the underlying issue and provide proper medication to treat the issue. Patients who are dehydrated may also be given IV fluids to correct electrolyte imbalance and alleviate other symptoms. If the cause of your digestive issues is not immediately identifiable, we will order tests to determine if you have gallbladder disease, ulcers, gallstones, or some other digestive condition.

Urinary Symptoms

Kidney and/or bladder infections can develop if bacteria get into the urinary tract. Common symptoms of a urinary tract infection include incontinence or situational-incontinence when you cough, sneeze, or engage in physical activity. If you experience pain or difficulty while urinating, or if you have noticed blood in your urine, schedule an appointment today. Doing so can prevent the condition or infection from worsening.

Cost of Care Transparency
at Heritage Urgent & Primary Care

A visit to the emergency room or hospital can cost a substantial amount of money and take much longer for you to receive the care you need. At Heritage Urgent & Primary Care, our board-certified providers can provide you with high-quality care in a timely and cost-effective manner. If you require health services and are concerned about the cost involved, please refer to the following overview of our clinic’s policies. If you have any additional questions concerning billing, insurance, and/or payment options, please contact our staff.

Insurance Participation
and Copays

At this time, we do not participate with any Medicaid plans. We do accept Medicare and are in-Network with most major insurance providers. Please note that HMO products are not in-network with our practice. As a courtesy, we file all claims as Primary Care and collect the Primary Care Copay as determined by your insurance plan.

If copays are required by your insurance plan, collection will take place prior to your appointment at check in. If you are not able to pay your copay at the time of service, it may be necessary to reschedule your appointment. It is also important to be aware that not all services are covered through insurance plans and if your insurance company determines that a service is not covered, the remaining balance is the patient’s responsibility. We encourage patients to pay balances at clinic level to avoid delay in account balance statuses.

Out-of-Network and
Self-pay Accounts

If you do not have active insurance coverage or if your provider is out of network, you will be responsible for the entire charge of the appointment and considered a self-pay account. Self-pay accounts follow a reduced fee schedule to make the cost for care affordable. Self-pay account balances are collected in full at the time of service. Please note that medical cost sharing plans do not qualify as healthcare coverage and will follow our self-pay fee schedule.

Further Payment and
Insurance Information

You will need to provide your current insurance information and bring your insurance card with you to your first visit. We also ask that you bring your card to subsequent visits so we can update your records as needed. Our staff will also help ensure that you maximize your insurance benefits and will file your claims and provide any necessary information to your insurance company.
