We provide chronic disease management support for a wide range of issues, which are detailed below.

Depression and Anxiety Issues

Depression and anxiety are two of the most common mental health conditions that we assist patients in managing and are more prevalent in patients who are suffering from other chronic diseases. All too often, patients and their family members assume that the underlying reason why the individual exhibits symptoms of depression and/or anxiety is due to their chronic disease. While this is sometimes the case, it is important to ensure that patients are being treated for both the medical illness and depression or anxiety at the same time. We provide a variety of supports in managing depression and anxiety through diagnosis, medical therapy, support for making lifestyle changes, and referrals for other therapeutic methods that have been shown to alleviate depression and anxiety, such as yoga, acupuncture, and/or psychological counseling.

Diabetes & Endocrine Problems

Patients who have diabetes and other endocrine problems are at an increased risk of heart disease and other serious health issues. We provide a wide range of services to ensure that you are managing your blood sugar and remaining in good overall health, including diagnosis, medical therapy, support for making lifestyle changes, such as diet and exercise plans that are designed to improve your overall health, education on what foods you should eat, what foods you should avoid, and what signs to look for to indicate that your blood sugar levels are off. We also provide similar supports for patients with endocrine issues, including obesity management, bone care, adrenal and pituitary management, and thyroid care.

Heart Disease & Circulatory Problems

Heart disease is a common and very serious medial condition that requires life-long management and regular check-ups. Although cardiovascular disease is the leading cause of death in the United States, patients who have this issue can still live a happy, healthy, active life provided they practice good self-care and are educated about the illness. We provide a variety of supports in managing heart disease and circulatory problems through diagnosis, medications, regular screenings, medical therapy, and support for making lifestyle changes, such as developing a proper diet and exercise plan.

Arthritis and Other Joint related Problems

Arthritis is a broad term that refers to joint disease or joint pain and is the leading cause of disability in America. Common symptoms of arthritis include stiffness, pain, swelling, and diminished range of motion, with symptoms ranging from mild to severe. We provide a variety of supports for arthritis and other joint-related problems, including diagnosis, medications, coordinating care with specialists, regular screenings, medical therapy, and support for making lifestyle changes, such as developing a proper diet and exercise plan, maintaining a healthy weight, and educating yourself on the disease and available treatment options.

Asthma, COPD and other Respiratory Problems

Although asthma and COPD are different diseases, they are often mistaken for one another because many patients who have COPD also have asthma. They also have many overlapping symptoms, which include shortness of breath, wheezing, and coughing. Unlike asthma, which can be managed with proper treatment, COPD is a progressive disease. We provide a variety of supports in managing both asthma, COPD, and other respiratory problems, including diagnosis, determining what triggers your asthma and helping you develop ways to avoid these triggers, medication, coordinating care with specialists, regular screenings, medical therapy, and support for making lifestyle changes. Support in making lifestyle changes is especially important for patients with COPD, as taking steps such as quitting tobacco and following exercise training and/or nutritional advice can significantly increase the quality of your life and prolong the need for surgical intervention.

Allergies and Sinus Problems

Allergic reactions can be triggered by a range of factors, including insect bites, medications, foods, and the environment. Reactions can be as mild as a rash or as serious as being life-threatening. In order to manage and avoid emergency situations, it is important to identify what you are allergic to and/or what type of asthma you have. We provide a variety of supports in allergy management, including diagnosis, regular screenings, medications, and medical therapy when needed. We also provide support for patients who have frequent sinus problems, like chronic sinus infections. Once the underlying cause of your sinus infection has been determined, we will provide you with recommended treatment plans designed to encourage drainage and open your sinuses. The most common treatment for a sinus infection is antibiotics and decongestants. It can also be helpful to perform home treatments to open the affected sinus and loosen mucus, such as facial steam or hot showers.

Skin Problems / Conditions

There are a range of skin conditions that affect patients from all backgrounds and of all ages. We provide evidence-based treatment for a wide range of skin conditions, including common skin care procedures, such as foreign body removal, ingrown toenails, suturing of minor wounds, wart removal, cyst, skin biopsy, and moles. We also provide medical dermatology services, such as skin cancer screening, treatment for rashes, psoriasis, eczema, dry skin, dandruff, fungal/bacterial infections, allergic reactions, and acnes. Whatever your issue may be, we are here to provide you with the care you need to address the issue, and if we are not able to address your problem, we can refer you to a board-certified dermatologist who can.

We offer a range of services, including management of chronic disease, ongoing illness follow-up care, and if necessary, referral coordination to high value specialists. Our clinic offers same day primary care appointments and immediate walk-in availability and can provide you and your family with a range of health services, including urgent, acute, immediate, and walk in health care needs. We also provide services during early morning hours and after hours and during the weekend. To ensure you receive high quality care, our office utilizes state of the art facilities and equipment, including a large procedural room for performing procedures such as orthopedic splinting and/or wound repairs. Our clinic can also take on-site digital x-rays and work with local imaging centers to perform a wide array of diagnostic exams, including CT scans and/or ultrasound. We also perform a broad range of laboratory services for onsite and send out labs, and can perform gynecologic testing and exams, pregnancy tests, urinalysis, vaccines, rapid flu tests, and strep tests.

Cost of Care Transparency
at Heritage Urgent & Primary Care

A visit to the emergency room or hospital can cost a substantial amount of money and take much longer for you to receive the care you need. At Heritage Urgent & Primary Care, our board-certified providers can provide you with high-quality care in a timely and cost-effective manner. If you require health services and are concerned about the cost involved, please refer to the following overview of our clinic’s policies. If you have any additional questions concerning billing, insurance, and/or payment options, please contact our staff.

Understand Insurance and Medicare Medicaid

All visits are billed under Primary Care and Primary Care Copay. We accept Medicare and are In Network with most major insurance providers and are happy to provide you with a list of accepted providers to ensure you are covered. If co-payments are required by your insurance plan, this will need to be paid prior to your appointment before check in. If you are not able to pay your co-payment, it may be necessary to reschedule your appointment. It is also important to be aware that not all services are covered through insurance plans. If your insurance company determines that a service is not covered, it will be your responsibility to pay the remaining balance.

Out-of-Network and
Self-pay Accounts

If you are not covered by insurance, or if your provider is out of network, you will be responsible for the entire charge of the appointment. If you are not able to pay the full balance of your bill, please contact our billing office at 866-557-2612. We are committed to ensuring that you receive the care you need and will do our best to help set up a payment plan to cover the costs of your care.

Further Payment and
Insurance Information

You will need to provide your current insurance information and bring your insurance card with you to your first visit. We also ask that you bring your card to subsequent visits so we can update your records as needed. Our staff will also help ensure that you maximize your insurance benefits and will file your claims and provide any necessary information to your insurance company.

To ensure you receive necessary care, we offer weekend, extended, and walk in flexibility, as well as 24-hour phone coverage for established Primary Care patients. Every effort will be made to attend to walk-in Urgent Care patients within 24 hours.
