Although playing sports is a great way to stay fit and have fun, it is important to make sure that you are in good physical health to engage in strenuous activity, especially if you already have a sports-related ailment. Whether your child is participating in basketball, football, baseball, soccer, gymnastics, or any other sport, it is important that they have received a sports physical to ensure they are in good physical health to do so and at minimal risk of injury. Additionally, most schools require an updated sports physical before children can participate.

Patient-Centered Medical Home model

What to Expect During a Sports Physical

The sports physical will begin with a review of your child’s medical history and a review of your family’s medical history. If necessary, additional testing may be performed to address any preexisting conditions or potential issues. After reviewing your child’s and family medical history, the sports physical will consist of the following:

  • Check weight
  • Measure height
  • Listen to lungs and hearts
  • Examine ears, nose, and throat
  • Test eyesight
  • Feel abdomen
  • Test strength and flexibility

In addition to gathering this information, this appointment is also an opportunity for you and your child to ask any questions you might have about playing sports and your health in general. If you do have any questions you would like to ask our staff, it may be helpful to write these down ahead of time so you remember to ask.

In most cases, the sports physical will determine that your child is cleared to perform in extracurricular events. If a health issue is identified, we will provide you with recommended treatment or medication options. If your child has a preexisting injury that places them at risk for additional injuries, we may recommend other treatments or exercises to help minimize their risk of additional injury and/or speed up the recovery process.

Cost of Care Transparency
at Heritage Urgent & Primary Care

A visit to the emergency room or hospital can cost a substantial amount of money and take much longer for you to receive the care you need. At Heritage Urgent & Primary Care, our board-certified providers can provide you with high-quality care in a timely and cost-effective manner. If you require health services and are concerned about the cost involved, please refer to the following overview of our clinic’s policies. If you have any additional questions concerning billing, insurance, and/or payment options, please contact our staff.

Insurance Participation
and Copays

At this time, we do not participate with any Medicaid plans. We do accept Medicare and are in-Network with most major insurance providers. Please note that HMO products are not in-network with our practice. As a courtesy, we file all claims as Primary Care and collect the Primary Care Copay as determined by your insurance plan.

If copays are required by your insurance plan, collection will take place prior to your appointment at check in. If you are not able to pay your copay at the time of service, it may be necessary to reschedule your appointment. It is also important to be aware that not all services are covered through insurance plans and if your insurance company determines that a service is not covered, the remaining balance is the patient’s responsibility. We encourage patients to pay balances at clinic level to avoid delay in account balance statuses.

Out-of-Network and
Self-pay Accounts

If you do not have active insurance coverage or if your provider is out of network, you will be responsible for the entire charge of the appointment and considered a self-pay account. Self-pay accounts follow a reduced fee schedule to make the cost for care affordable. Self-pay account balances are collected in full at the time of service. Please note that medical cost sharing plans do not qualify as healthcare coverage and will follow our self-pay fee schedule.

Further Payment and
Insurance Information

You will need to provide your current insurance information and bring your insurance card with you to your first visit. We also ask that you bring your card to subsequent visits so we can update your records as needed. Our staff will also help ensure that you maximize your insurance benefits and will file your claims and provide any necessary information to your insurance company.
