Heritage Payment/Insurance Info

At Heritage Urgent & Primary Care, our board-certified providers can provide you with high-quality care in a timely and cost-effective manner. If you require health services and are concerned about the cost involved, please refer to the following overview of our clinic’s policies.

  • Insurance: All visits are billed under Primary Care and Primary Care Copay. We accept Medicare and are In Network with most major insurance providers. We are happy to provide you with a list of accepted providers to ensure you are covered. If co-payments are required, this will need to be paid prior to your appointment before check in. If your insurance company determines that a service is not covered, it will be your responsibility to pay the remaining balance. Our staff will help ensure that you maximize your insurance benefits and will file your claims and provide any necessary information to your insurance company.
  • Out-of-Network/Self-pay Accounts: If you are not covered by insurance, or if your provider is out of network, you will be responsible for the entire charge of the appointment. If you are not able to pay the full balance of your bill, please contact our billing office at 866-557-2612. We are committed to ensuring that you receive the care you need and will do our best to help set up a payment plan to cover the costs of your care.

STD Testing & Pelvic Exams

DOT Physicals

Commercial drivers and other employees who are designated as safety-sensitive by the Department of Transportation are required to have a DOT Physical, a specialized type of physical whose guidelines are mandated by the Federal Motor Carrier Safety Administration (FMCSA).

Vaccines & Immunizations

Vaccines contain organisms that have been weakened or are dead and are administered to provoke an immune system response which results in the patient developing an immunity from the disease. Once vaccines are administered, the body goes through an immunization process, which prepares your immune system to fight diseases that are part of the administered vaccine immunizations.

Sore Throat

A sore throat (pharyngitis) can present in a variety of ways, including hoarseness, tonsil swelling, pain or difficulty swallowing, and scratchiness. A sore throat can also be associated with other symptoms, such runny nose, nausea, vomiting, chills, fever, headaches, and body aches.

Sinus Infection

Sinus infections occur when your sinuses become swollen or inflamed. This swelling or inflammation may occur for a variety of reasons, including allergies, colds, nasal polyps, and/or anatomic conditions. Once a blockage has developed in your sinuses, bacteria begins to grow, which can cause an infection.

Strep Test & Strep Throat

Strep Throat (streptococcal pharyngitis) occurs when your tonsils and throat become swollen or inflamed due to an infection. Streptococcal bacteria is highly contagious and easily spread, typically by the infected individual sneezing or coughing.

Nausea, Vomiting & Diarrhea

Nausea is the feeling that precedes vomiting, but not the actual act. Vomiting occurs when the contents of your stomach are forced up your digestive track and out your mouth. Diarrhea is classified as three or more episodes of watery and loose bowel movements in a day. Like nausea and vomiting, diarrhea can be caused by a range of factors, such as food poisoning, influenza, morning sickness, and a range of other health conditions.

Food Poisoning

Food poisoning is caused by eating food that is contaminated, most commonly by infectious organisms, such as parasites, viruses, and bacteria. Contamination can happen at home if food is incorrectly cooked or handled or during the food production process or during processing, storing, or shipping.

Pap Smear

Starting at the age of 21, Pap smears should be performed every three years for women between the ages of 21 to 65, even if you are not sexually active. This exam is an effective method to test for cervical cancer and can identify cervical cell changes that may indicate the development of cancer at a later date.

Ear Infections

An ear infection develops due to a virus or bacterium in the middle ear. Ear infections are more common in children than adults and are often the result of another illness, such as allergies, flu, or the cold which cause swelling and congestion of the throat, nasal passages, and eustachian tubes.

Laceration, Suturing & Wound Care

A laceration is categorized as a deep tear or cut in the skin or flesh that is typically jagged or torn. Unlike most scrapes and minor cuts, which can heal naturally, lacerations will require suturing and wound care to heal.

Stitches

Pink Eye

Pink eye (conjunctivitis) occurs when the transparent membrane that covers the white of your eyeball and lines your eyelid becomes infected or inflamed, resulting in the white of your eyes to appear pink or reddish. Pink eye can occur due to bacteria, viruses, or allergens and may affect one or both eyes.

Poison Ivy

Poison ivy rash occurs because of urushiol, an oil that is present in the stems, roots, and leaves of the poison ivy plant. When exposed to the plant directly (or secondarily) by direct contact, gardening tools, or pets, a reaction may occur.

Dehydration

When you lose more body fluids than you are taking in, this imbalance causes dehydration. Dehydration can occur for a variety of reasons, including exercise, and high environmental temperatures, but is most common among individuals with the stomach flu, which causes diarrhea and vomiting, as well as infants, elderly adults, and people with chronic illnesses.

Foreign Body Removal

When a foreign object or objects becomes lodged in a part of the body, it will be necessary to remove the object. Ears are the most common areas for foreign body removal, with the majority of the foreign bodies being insects, such as flies, moths, ants, and cockroaches. Adults tend to not know how the object became lodged, whereas children often place the objects voluntarily.

Orthopedic Fracture & Dislocation

A fracture (broken bone) can occur a variety of ways, including due to osteoporosis, repetitive stress injuries, tumors, or most commonly, due to an accident. When a joint becomes displaced from its normal position, this is referred to as dislocation. Joint dislocation can also be caused by a variety of things, but is most commonly the result of a fall, blow, or other kind of trauma.

Bronchitis

Bronchitis occurs when the tubes that carry air in your lungs become irritated or inflamed, usually resulting in coughing thick, discolored mucus. There are two types of bronchitis: acute and chronic.

Abscess Incision & Drainage

Patients who have developed an abscess will require incision and drainage (I&D). Because the bacteria in an abscess are in a pocket, antibiotics are not able to reach and destroy the bacteria. Once the abscess has been opened, the pressure will immediately be alleviated and the healing process can begin.

Respiratory Illnesses & Infection

Respiratory infections are any infection of the lungs, airways, throat, or sinuses. Respiratory infections are distinguished as being upper or lower depending on what area they impact. Respiratory infections that impact the neck up are classified as upper respiratory infections, whereas respiratory infections that impact the chest area are classified as lower respiratory infections.

Biometric screening is a method of developing a clinical assessment of key health measures and are often done in conjunction with a health risk assessment questionnaire, which gathers information concerning an individual’s health status and lifestyle behaviors.

Urinary Tract Infections

Urinary tract infections (UTIs) develop when bacteria enters the urinary tract and travels to the bladder, where it can spread to the kidneys. Women are more likely to experience a UTI, but it is a common condition that affects both men and women and all age groups.
