Commercial drivers and other employees who are designated as safety-sensitive by the Department of Transportation are required to have a DOT Physical, a specialized type of physical whose guidelines are mandated by the Federal Motor Carrier Safety Administration (FMCSA). DOT physicals are valid for 24 months, although individuals who have recurring ailments may need to schedule DOT physicals with more frequency. Once 24 months has passed, another DOT physical is required in order for the employee to be considered compliant and to continue working.

To help ensure that the physical runs as smoothly as possible, it is important to bring the necessary documentation and information, including things like your current list of medications, along with dosages and dosage regimen. Additionally, drivers with hearing or vision issues must bring their hearing aids and/or contacts or eyeglasses.

Patient-Centered Medical Home model

What To Expect at a D.O.T. Physical

A standard DOT Physical exam will generally begin with a review of your medical history and discussion of any current health issues or concerns. Next, we will test your vision, to ensure that you have at least 20/40 acuity in each eye and a 70” peripheral in the horizontal meridian. Next, we will test your hearing to ensure that you are able to perceive a “forced whisper”. This test can be performed with or without hearing aids. Next, we will examine your blood pressure and pulse rate to test for irregular heartbeats or high blood pressure. Finally, a urinalysis test will be administered to check for blood, sugar, and protein levels. If necessary, a 5-panel DOT controlled substance screen will also be performed, which will test for marijuana (THC), phencyclidine (PCP), cocaine (COC), amphetamines (AMP), and opiates (OPI).

A physical examination will also occur, which will include examination of the following:

  • General appearance
  • Eyes (indication of macular degeneration, glaucoma, cataracts, etc.)
  • Ears (indication of perforated ear drums, scarring of tympanic membrane, etc.)
  • Mouth and throat (indication of difficult swallowing or breathing)
  • Heart (pacemaker, extra sounds, murmurs, etc.)
  • Chest and lungs and chest (indication of cyanosis, impaired respiratory functions, abnormal breathing, etc.)
  • Abdomen and Viscera (indication of muscle weakness, viscera, enlarged liver)
  • Vascular (indication of varicose veins, carotid, abnormal pulse)
  • Extremities (limb impaired)
  • Genito-urinary (hernias)
  • Spine and musculoskeletal (tenderness, limitation of movement, previous surgery, etc.)
  • Neurological (indication of asymmetric deep tendon reflexes, ataxia, impaired equilibrium, coordination or speech pattern)

Cost of Care Transparency
at Heritage Urgent & Primary Care

A visit to the hospital for a DOT physical can cost a substantial amount of money and take much longer for you to receive the care you need. At Heritage Urgent & Primary Care, our board-certified providers can provide you with high-quality care in a timely and cost-effective manner. If you require health services and are concerned about the cost involved, please refer to the following overview of our clinic’s policies. If you have any additional questions concerning billing, insurance, and/or payment options, please contact our staff.

Understand Insurance and Medicare Medicaid

All visits are billed under Primary Care and Primary Care Copay. We accept Medicare and are In Network with most major insurance providers and are happy to provide you with a list of accepted providers to ensure you are covered. If co-payments are required by your insurance plan, this will need to be paid prior to your appointment before check in. If you are not able to pay your co-payment, it may be necessary to reschedule your appointment. It is also important to be aware that not all services are covered through insurance plans. If your insurance company determines that a service is not covered, it will be your responsibility to pay the remaining balance.

Out-of-Network and
Self-pay Accounts

If you are not covered by insurance, or if your provider is out of network, you will be responsible for the entire charge of the appointment. If you are not able to pay the full balance of your bill, please contact our billing office at 866-557-2612. We are committed to ensuring that you receive the care you need and will do our best to help set up a payment plan to cover the costs of your care.

Further Payment and
Insurance Information

You will need to provide your current insurance information and bring your insurance card with you to your first visit. We also ask that you bring your card to subsequent visits so we can update your records as needed. Our staff will also help ensure that you maximize your insurance benefits and will file your claims and provide any necessary information to your insurance company.
