An emergency room visit for many acute illnesses that can be managed in our urgent care can cost you substantial time and money. That is why we are committed to effectively and efficiently managing a host of acute & sub-acute medical illnesses. As your primary care provider, we can ensure appropriate and ongoing illness follow-up care, management of chronic disease, and coordinate referrals to high-value specialists if necessary. Most insurance is accepted; self-pay patient discount and discounted lab charges through Quest Diagnostics.
Our Mission & Values
Patient-Centered Medical Home
As the new era of healthcare continues to change and federal guidelines come into effect, providers and patients need to work together to effectively coordinate care so that optimal health care can be provided.
In the old healthcare system, patients would seek medical attention when they were injured or not feeling well. In the new era of healthcare, patients need to take more responsibility for their health care by seeing the provider before they are sick. Providers and patients are being held more accountable for the care being provided. Some responsibilities of the providers and patients are listed below.
As providers and partners in your health care team, we will:
- Ask you to take part in your health care
- Listen to your questions and concerns to give an appropriate response.
- Help you set health goals and create action plans that are easy to understand
- Refer you to specialists and hospitals as needed & track the care received from other providers
- Provide medical care while the office is open, by offering appointments at times when you can come in
- Make sure you have a good understanding of lab results, medication management, & treatment goals
- Help you get support services when you need them
- Provide disease-specific educational materials to assist in your self-management
- Stay in contact with you as your partner in care
As a patient and partner of Heritage Urgent & Primary Care, I will:
- Take an active role in my health care
- Bring all the questions I have to my appointments and ask my provider about things I don’t understand
- Tell my provider when things are affecting my life and ask for support services when I need them
- Help my provider create my action plan and track my progress
- Call my provider first with medical problems, unless it is a medical emergency.
- Let your office know when I get care somewhere else
- Bring all medicines, supplements, and herbal or holistic products I use to my appointments
- Fill my prescriptions on time, use them as prescribed, and tell you of any problems
- Keep all scheduled appointments with my provider and other specialists I see
- Make sure all of my insurance and personal information stays up to date
What Is a Patient-Centered Medical Home?
What is a Patient Center Medical Home?
A patient-centered medical home is the new era of medicine. It can be described as a model or philosophy of primary care that is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety. It has become a widely accepted model for how primary care should be organized and delivered throughout the health care system, and is a philosophy of health care delivery that encourages providers and care teams to meet patients where they are, from the simplest to the most complex conditions. It is a place where patients are treated with respect, dignity, and compassion, and enables strong and trusting relationships with providers and staff. Above all, the medical home is not a final destination; instead, it is a model for achieving primary care excellence so that care is received in the right place, at the right time, and in the manner that best suits a patient’s needs.
Patient Center Medical Home
Features of the Medical Home
- Patient-centered: A partnership among practitioners, patients, and their families ensures that decisions respect patients’ wants, needs, and preferences, and that patients have the education and support they need to make decisions and participate in their own care.
- Comprehensive: A team of care providers is accountable for a patient’s physical and mental health care needs, including prevention and wellness, acute care, and chronic care.
- Coordinated: Care is organized across all elements of the broader health care system.
- Accessible: Patients are able to access services with shorter waiting times for urgent needs, enhanced in-person hours, and “after-hours” telephone access to a member of the care team.
- Committed to quality and safety: Clinicians and staff enhance quality improvement to ensure that patients and families make informed decisions about their health.
Patient-Center Medical Home
Medical Home is not Fragmented Medical Care
In the past, you may have received your care at many different provider locations without a common medical record, and medical history could become fragmented. As a result, it’s difficult for your primary care provider to get a picture of your health. Therefore, health care is costly and inefficient. This type of health care is discouraged and is in violation of standards for our practice, which is a patient-centered medical home.
Heritage Urgent & Primary Care encourages all of our primary care patients to adopt this new standard in medicine by
- Reporting that Heritage Urgent & Primary Care is your primary care provider when receiving medical services at other facilities or hospitals.
- Notify us of services received at other facilities or hospitals, and provide discharge orders when appropriate.
If you do not have a primary care provider or have not seen your primary care provider for the past 3 years. Your insurance may recognize Heritage Urgent & Primary Care as your new provider.
